1. Affidavit of Renunciation of Philippine Citizenship (2 copies)
2. Passport Size photographs (size 3.5cm x 4.5cm) with white background (2 copies)
3. Original Passport and 2 photocopies of Passport data page
4. Original and Photocopy of Birth Certificate issued by Philippine Statistic Authority (PSA)
5. Original and Photocopy of the NOTICE OF APPROVAL/CERTIFICATE OF MALAYSIAN CITIZENSHIP from the Malaysian Immigration
6. Accomplished Application Form for Renunciation of Philippine Citizenship Form (2 copies), form will be provided at the Embassy
7. FEE of RM260.00 (cash only)

Submission of the requirements and duly-accomplished forms at the Embassy is during working hours in the morning only, 9 AM - 12 NN, Mondays to Fridays, except weekend and holidays. No appointment is needed. 

Personal appearance is required. Release of documents is after 3 working days. For expedite processing, an additional fee of RM 52.00 will be charged per document.
