REQUIREMENTS FOR CERTIFICATION OF NON-ISSUANCE OF PHILIPPINE PASSPORT (FOR JPN)

1. Statutory Declaration in English stating that applicant has never applied for a Philippine passport nor had his/her birth reported at the Philippine Embassy, for those born in the Philippines, state how you entered Sabah withouth using a Philipine passport, stamped by a Commisioner of Oaths or Notary Public.

2. Attached a copy of any Malaysian identity documents.

3. Personal appearance at the Embassy.

4. Fee of RM 130.00 

If you are born from Sabah and intend to send the Statutory Declaration via poslaju, please be guided by the following requirements:

1. Statutory Declaration notarized by any Commissioner for Oaths, Malaysia;

2. Said Notarized Statutory Declaration should then be Authenticated by an Officer at the Sabah Regional Office.
