The NBI clearance form is not a consular document. However, the Philippine Embassy in Kuala Lumpur assists Filipino nationals and foreign nationals who had lived in the Philippines, in applying for the National Bureau of Investigation (NBI) clearance in terms of fingerprinting on the NBI clearance form no. 5. 

The applicant shall then make arrangements to send the duly accomplished NBI clearance form no. 5, together with an authorization letter, to the applicant’s representative in the Philippines who will process the clearance with the NBI in the Philippines.

Information and contact details of the NBI personnel in the Philippines shall be provided at the Embassy during the filing of application.

Requirements:

1. Two (2) 2x2 white background picture;

2. Original and photocopy of passport data-page, valid Malaysian visa, or any government-issued ID.

 

Consular fee: RM130.00

Processing period: 1 day

Submission time: 1:30 pm - 3:30 pm only

Important:

Personal Appearance of the applicant is mandatory. Clients are reminded to bring their complete sets of documents, including originals and photocopies, for their own convenience.

 

Application for notarial and authentication services through Pos Laju may be allowed, so long as the document has been notarized/authenticated by MOFA or Commissioner for Oaths. The surrender of Philippine passport, however, requires personal appearance.
