 REQUIREMENTS FOR REPORT OF DEATH

 

1.  One (1) completely and correctly accomplished original Report of Death Form;

2. Death certificate issued by the Jabatan Pendeftaran Negara (National Registration Department). Note: if the death certificate is in Bahasa Malaysia, it must be supported by notarized English translation duly authenticated by the Ministry of Foreign Affairs, Putrajaya.

3.  Copy of passport or old ID of the deceased as proof of Philippine citizenship

4.  Clear copy of PSA birth certificate of the deceased

5.  Clear copy of PSA/JPN marriage certificate of the deceased (if applicable)

6.  Copy of valid government ID of the nearest of kin (who will file the ROD as informant)

7.  Police report issued where the death occurred

8.  All available documents pertinent to the death of the deceased:

a.      No objection certificate;

b.      Mortuary certificate

c.      Embalming certificate / cremation certificate;

d.      Certificate of sealing of casket;

e.      Transit permit/export permit;

f.       Letter of acceptance issued by the family member in the Philippines;

g.       Post mortem / autopsy report.

9.Affidavit of delayed registration of Report of Death (for the death that occurred more than a year)

Affidavit of Delayed Registration – RM130.00

Reminder:

  • Please bring the Original and five (5) photocopies of all the documents for the Report of Death.
  • Affidavit/s is to be notarized by the Consul of the Philippines in Malaysia. Notarization from a Commissioner for Oaths is not necessary.
