REQUIREMENTS FOR REPORT OF MARRIAGE

1) One (1) duly accomplished original Report of Marriage Form

▪ All entries must be printed or handwritten legibly
All entries must be at the time of marriage

2) Four (4) copies of the Marriage Certificate from the Jabatan Pendaftaran Negara (JPN) or the National Registration Department (NRD), duly authenticated by the Kementerian Luar Negeri or the Ministry of Foreign Affairs in Putrajaya;

For applicants married in Muslim rites - Four (4) copies of Marriage Contract with official English translation (if applicable);

3) Four (4) copies of passport/s of the Filipino party/parties;

4) Four (4) copies of Identification Card (IC) or passport of the foreign spouse;

5) For Filipino Muslim converts:

Four (4) copies of Certificate of Conversion to Islam and annotated Birth Certificate from PSA (if available);

6) For divorce/annulled foreign husband:

For Malaysians: Four (4) copies of updated Marriage Record issued by JPN and divorce decree both must be duly authenticated by the Kementerian Luar Negeri or the Ministry of Foreign Affairs.

For other nationalities: Four (4) copies of Divorce decree with official English translation (if not in English);

7) For widow/widower party - Four (4) copies of Marriage Certificate and Death Certificate of previous spouse with official English translation (if not in English);

8) Two (2) recent colored passport size photos of contracting parties;

9) Four (4) copies of Legal Capacity to Contract Marriage (LCCM) issued to the Filipino national prior to marriage (if available);

10.) Affidavit of Delayed Registration (required only for applicants who are married for one (1) year or more than a year)

11.) Personal appearance of both parties is required.

REMINDER: PLEASE BRING ALL THE ORIGINAL DOCUMENTS.

ADDITIONAL DOCUMENTS MAY BE REQUIRED AS DEEMED NECESSARY

CONSULAR FEE: RM 130.00

PROCESSING PERIOD: 3 Working Days

For applicants who are based in Sabah and East Malaysia, all documents issued by the Sabah government must be authenticated by the Ministry of Foreign Affairs in Sabah, while other documents must be notarized by a Commissioner of Oaths/Justice of Peace and duly authenticated by the Ministry of Foreign Affairs in Sabah.
