REQUIREMENTS FOR REPORT OF DEATH



 CHECKLIST
 

Confirmed online appointment (http://philembassyklappointment.biz)

  Duly accomplished Report of Death (ROD) Form, all entries may be typewritten or printed handwritten (4 copies);
  Death Certificate issued by the Jabatan Pendeftaran Negara (National Registration Department of Sabah);
Note: If the Death Certificate is in Bahasa Malaysia, it must be supported by notarized English translation duly authenticated by the Ministry of Foreign Affairs, Putrajaya.
  Copy of passport or old ID of the deceased; Copy of latest visa of the deceased (if applicable);
  Clear copy of PSA/JPN Birth Certificate/Report of Birth of the deceased;
  Clear copy of PSA/JPN Marriage certificate of the deceased (if applicable);
  Copy of valid government ID of the nearest of kin / informant / undertaker (one who will file for the ROD as informant);
  Police report issued where the death incurred; and,
  Other documents available pertinent to the death of the deceased.

☐ No Objection Certificate (Issued by the Embassy);

☐ Mortuary Certificate

☐ Embalming Certificate / Cremation Certificate;

☐ Certificate of Sealing of Casket;

☐ Transit Permit/Export Permit;

☐ Letter of acceptance issued by the family member in the Philippines;

☐ Post Mortem / Autopsy report;

☐ Flight details or itinerary (if for repatriation).

  Affidavit of Delayed Registration of Report of Death will be required if the death occurred more than a year already (if applicable).
