- present address/location sa Malaysia
- Ano ang hinihingi niyong tulong?
- Kung may mga kapamilya kayong kasama, pakilagay din ang kanilang detalye.
- Paki-attach ang copy ng inyong mga passport.

2. Magpadala ng text message o via Whatsapp/Viber sa +60147201295
(hindi nakakatanggap ng tawag / text lamang).

Sisikapin ng Embahada na ipaabot ang lahat ng nararapat na tulong nang hindi na kinakailangang magpunta sa Embahada, kung maaari.

REQUIREMENTS FOR CONVERSION OF PHILIPPINE DRIVER’S LICENSE TO MALAYSIAN DRIVER’S LICENSE 

Those who wish to convert their Philippine Driver's License to Malaysian driver’s license are required by Jabatan Penangkutan Jalan (JPJ) Malaysia or the Road Transport Department of Malaysia to submit a certificate from the Philippine Embassy.

The Embassy requirements for the issuance of the certificate are as follows:
1. Original and photocopy of valid Philippine Non-Professional or Professional Driver’s License
2. Original and photocopy of the Official Receipt from the Land Transportation Office (LTO)
3. LTO Certification on the issuance of LTO driver’s license duly authenticated by the Department of Foreign Affairs (DFA) – Authentication Division

4. Original and photocopy of valid Philippine passport and Malaysian visa

 
 

Processing fee: RM130.00 (cash only)

REQUIREMENTS FOR SPECIAL POWER OF ATTORNEY

 
  1. Original and photocopy of data page of passport and valid visa
  2. Duly accomplished affidavit of special power of attorney
 

Notarial fee: RM130.00 (cash only)
Submission time: 9:00 am - 12:00 nn only
Processing period: 3 working days
