  Birth / Marriage / Death certificate issued by the Jabatan Pendeftaran Negara (JPN);
  Baptismal Certificate;
  School records (Form 137 / Transcript of Record);
  PSA / JPN Marriage Contract, if applicable;
  PSA Birth Certificate of children, if any;
  PSA Birth Certificate of siblings;
  PSA Birth Certificate of mother/father;
  Notarized Affidavit of Discrepancy;
  Notarized letter of the authorized person who will file.
