REQUIREMENTS FOR REPORT OF BIRTH OF CHILDREN BORN ABROAD OF FILIPINO PARENT/S

1) One (1) duly accomplished Report of Birth Form

  •   All entries may be typewritten or handwritten legibly;
  •   Entries on the Report of Birth form must be at the time of birth of the child/subject
  •   Entry in the “Place of Birth” should be the city or state + country where the child is born – i.e. SELANGOR, MALAYSIA; KOTA KINABALU, SABAH

2) Birth Certificate must be issued by Jabatan Pendaftaran Negera (JPN) and duly authenticated by the Ministry of Foreign Affairs, Putrajaya - (4 copies);

3) Proof of Identity of applicant – (4 copies);

4) Marriage Contract of parents (if applicable) - (4 copies);

  •  If Marriage Certificate is in Bahasa Malaysia, it must be accompanied by an official English translation.
  •  If Marriage Certificate is in a foreign language, it must be accompanied by an official English translation from the respective embassy/consulate of the country where the document was issued.
  •   If married in the Philippines, submit PSA issued Marriage Certificate.

5) Proof of citizenship of parent/s - Parent/s passport/s (4 copies);

  •   Additional: PSA birth certificate of mother if parents are not married and the child will use the surname of the mother

6) Two (2) passport size photo of applicant;

7) Affidavit of Delayed Registration of Birth, if the child is aged one year old and above – (4 copies);

8) Four (4) originally signed Affidavit of Legitimation by Subsequent Marriage, if the child was born before the marriage of the parents – (4 originally signed); and

  •   Four (4) photocopies of parents’ passport/valid ID;
  •   Original and four (4) photocopies of PSA issued CENOMAR of Filipino parent/s

* 9.)  For applicants of legal age, submit old documents issued prior to the registration of birth.

e.g., old ID’s, school records, other documents as proof of identity and citizenship.

*Additional Requirements for the Report of Birth of children whose parents are not married but opt to use the surname of father:

Personal appearance of both parents is required upon filing of the following affidavits and to be submitted upon application of the Report of Birth:

To be accomplished/executed by the MOTHER:

1) Four (4) original Affidavit to Use the Surname of Father (AUSF); and

2) Four (4) photocopies of mother’s passport and PSA birth certificate.

To be accomplished/executed by the FATHER:

1) Four (4) original Acknowledgement of Paternity (AAP); and

2) Four (4) photocopies of father’s IC or passport.

CONSULAR FEES:

  Report of Birth – RM130.00 

Affidavit of Admission of Paternity – RM130.00

  Affidavit of Delayed Reg. – RM130.00

Affidavit to Use Surname of the Father – RM130.00

  Affidavit of Legitimation by Subsequent Marriage – RM130.00

Reminders: 

  •  Personal appearance of minor applicants (17 years old and below) is not required.
  • Bring all original documents and clear photocopies.
  •   Additional documents may be required as deemed necessary.
  •   Only Affidavit of Delayed Registration may be notarized by a Commissioner of Oaths. Other Affidavit/s stated above is to be notarized by the authorized officer of the Embassy
