REQUIREMENTS FOR SURRENDER OF PHILIPPINE PASSPORT

 

 

1. Duly accomplished statutory declaration stating that the individual wants to voluntary surrender their Philippine passport

2. Original with one (1) photocopy of the certificate of Malaysian citizenship (Sijil Kewarganegaraan Malaysia) Form G (Borang G)

3. Original PSA birth certificate

(If details are unreadable, a clear copy of the birth certificate from the local civil registrar is required);

4. Surrender applicant’s current Philippine passport.

If the applicant’s Philippine passport was lost, the applicant must submit an affidavit of loss explaining the circumstances the passport was lost, along with a police report (with English translation);

 

Processing fee: RM130.00 (cash only)

(An additional RM130.00 will be charged for Affidavit of Loss, if applicable

Submission time: 9:00 am - 12:00 nn only

Important:

Personal appearance of the applicant is mandatory.

RETENTION AND RE-ACQUISITION OF PHILIPPINE CITIZENSHIP RA 9225

 

Former Filipinos may re-acquire their Philippine citizenship through Republic Act (RA) 9225 or the Citizenship Retention and Re-acquisition Act. This law, which took effect on 17 September 2003, declares that former natural-born Filipino citizens who acquired foreign citizenship through naturalization are deemed to have not lost their Philippine Citizenship under conditions provided in the Act, a former Filipino can re-acquire Philippine Citizenship by taking an Oath of Allegiance to the Republic of the Philippines.

  REQUIREMENTS FOR PRINCIPAL:

Duly accomplished, verified petition/application form for Retention and Re-acquisition of Philippine Citizenship;

Four(4) pieces passport size photograph with white background and must be taken within the last three months from the date of application;

Original and DFA authenticated copy of proof of natural-born Philippine citizenship (e.g. Philippine Birth Certificate, cancelled Philippine passport, Voter’s Identification Card, Philippine Marriage Certificate indicating the Philippine citizenship of the petitioner) or any other Philippine-issued official document proving natural-born Philippine citizenship;

Photocopy of Certificate of Naturalization or other naturalization documents, with English translation if written in other foreign language, or affidavit stating how and when foreign citizenship was acquired;

Original and photocopy of valid foreign passport;

DFA Authenticated Marriage Certificate, if married;

DFA Authenticated Death Certificate, if widowed; and

DFA Authenticated Court Decision to nullify the marriage, if annulled.

  REQUIREMENTS FOR EACH DEPENDENT CHILD:

Accomplished Supplement for Dependent Form;

Four(4) pieces passport size photograph with white background and must be taken within the last three months from the date of application;

Photocopy of Birth Certificate of Dependent Child; and

Photocopy of valid foreign passport bio-page of Dependent Child.

Requirements for AUTHENTICATION OF DOCUMENTS SIGNED IN MALAYSIA

 

1) Get confirmed online appointment of the Embassy (https://philembassyklappointment.biz/app/public/)

2) Present the original documents to a Commissioner of Oaths / Notary Public in Malaysia for notarization;

3) Submit the notarized documents to the Consular Section of Wisma Putra (Ministry of Foreign Affairs) for authentication;
