*Submit three (3) photocopies of all the requirements and bring all original documents.
1. Three (3) original Petition Form;
2. PSA copy of the Birth Certificate/ Report of Birth;
3. Earliest school records/ earliest school documents;
4. Baptismal Certificate and other documents issued by religious authorities;
5. Other supporting documents reflecting the correct date/month of birth and sex;
6. Clearance or Certification that the document owner has no pending administrative, civil or criminal case or no criminal record which shall be obtained from the following:
a. Employer (if employed)
b. National Bureau of Investigation (NBI)
c. Philippine National Police
7. Affidavit of Publication from the Publisher and a copy of newspaper clippings;
8. For correction of sex, the petition shall be supported with a medical certification issued by an accredited government physician stating that the petitioner has not undergone sex change or sex transplant.

3. Original and three (3) photocopies of Baptismal Certificate and other documents issued by religious authorities;
4. Original and three photocopies of School Records (form 137/ transcript of records), if applicable;
5. Original and three photocopies of PSA/JPN Marriage Certificate, if applicable;
6. Original and three photocopies of PSA birth certificate of children, if any;
7. Original and three photocopies of PSA birth certificate of siblings, if applicable;
8. Original and three photocopies of PSA birth certificate of parents, if applicable;
9. Original and three photocopies of Notarized Affidavit of Discrepancy;
10. Notarized authorization letter if petition will be submitted through a representative.
11. For Petition for change of first name, additional to the requirements mentioned above, submit the following:
a. Barangay/Police or NBI clearance;
b. Certificate from employer stating there is no administrative or any pending case, whatsoever;
c. Affidavit of unemployment, if unemployed;
d. Affidavit of Publication from the Publisher and a copy of newspaper clippings.

Reminder:
- Please bring the Original and five (5) photocopies of all the documents for the Report of Death.
- Affidavit/s is to be notarized by the Consul of the Philippines in Malaysia. Notarization from a Commissioner for Oaths is not necessary.
